Default Problem with Phil-a-Form

I have already posted this problem in the PAF forums but have gotten no reply. My original message is as follows:

I have installed Phil-a-form along with the mambots. Although I did have to manually ftp the philaform.xml file up into the mambots directory in order to fix an error message in the admin > Mambots page. I'm really not sure what's wrong and would greatly appreciate anybody's help.
I need to get this resolved ASAP. I have installed the latest MSAS, followed by Zend Optimizer on my local machine. PAF works perfectly fine there so I am inclined to believe that it may be a setting that is misconfigured on my live site. I have not been able to discern any difference between the two setups. The components folder for each has permissions of '755'.

I'm not sure why I have not been getting your emails. I got the one from your sales address on Tuesday. I will look into it. I have submitted my info on your form and look forward to a resolution.

To all readers,

I would also like to make it quite clear that I am not, as I have seen other do, attempting to slander or trash Phil's name or reputation in public forum. As you can see from the above posts we were having somewhat of a communication problem. That seems to be resolved now and we are moving forward with a resolution.

Hi
I had the same problem as you with the front page coming up.

Was this when you bound to a table and then preview? or was it when you bound to a table?

I managed to get it to work by incrementing the number of the new table name in the bind to table page, and this then bound properly and then allowed me to preview.

I am also having problems with philaform. i am running version 1.0.0.0 (i got it today) and get this error on version 4.5.2 mambo when submitting form. i am having the user choose which email address to send the form too with checkboxes. everthing is published...
