how do i add an image to the left side of Main Menu, User Menu, Login Form, etc... ?

Put the imagepath in front of the description in admin section.
May be before you need to change with phpmyadmin the value from 250 to 255.

example: <img src="/images/yourpic.jpg"> User Menu

In V1.4 it works fine and i think in the newer versions it will work too.
