I've seen a lot of information about Art*Forms but can't seem to find a download ANYWHERE that isn't a broken link. Argh! Does it even work on Joomla! anyway? Can someone point me to a working link for this? (The website of the developers of Art*Forms was just hopeless - I couldn't find a true link to download it, just a bunch of sponsored ads. Grr.)

If someone could point me to the code in Joomla! that controls the form I could just change that, perhaps. I just need to add a couple more text fields, and maybe a checkbox or radio button or two.
